Swedish Medical Center Clinical Social Worker - Swedish Cancer Institute Integrated Care in Seattle, Washington
Swedish Medical Center is looking for a Clinical Social Worker (Per Diem, Days) to work in the Swedish Cancer Institute of Integrated Care at Swedish First Hill in Seattle, WA.
The Social Worker is responsible for providing care coordination, identification, assessment and intervention ofpatient / family needs including coordination of cancer support resources,referral to appropriate resources and emotional supportive counseling topatients and their families receiving their care at the Swedish CancerInstitute. Facilitates educational classes and supportgroups. In addition, participates as a social work practicum instructorfor interns. Must be able to commute to multiple Swedish Medical Center sitesand affiliate locations to provide oncology social work services.
In this position you will:
Complete initial assessments on all patients admitted
Develop an individualized treatment plan for the patient's anticipated stay
Arrange post-discharge planning
Coordinate treatment activities with other disciplines and team members
Collect, analyze and repeat quality management data for the social work department
Required qualifications for this position include:
Master's Degree in Social Workfroman accredited School of Social Work
Licensed Social Work Associate - Advanced (LSWAA) or Licensed Social Work Associate - Independent Clinical (LSWAIC)
Graduatedsocial work interns through the Swedish Cancer Institute internship program areeligible to apply. Otherwise, a minimum of two years post-graduate experiencein providing services to clients who have chronic and acute illness in a healthcare setting, preferably oncology
Preferred qualifications for this position include:
Licensed Independent Clinical Social Worker (LICSW) or Licensed Advanced Social Worker (LASW) with the Washington State Department of Health
Education/experience in geriatric and adult populations
Education/experience with a disabled population
Utilization management experience
Social work/discharge planning experience in a rehabilitation unit or acute care hospital
