Swedish Medical Center Social Worker - Heart and Vascular Clinic in Seattle, Washington


Swedish Medical Group is looking for a Social Worker (1.0, Days) to work in the Heart and Vascular Clinic at Swedish Cherry Hill in Seattle, WA.

The Social Worker is responsible for providing medical related social services to patients and families. These services include psychosocial and discharge needs assessments, planning and intervention.

In this position you will:

  • Communicate effectively with patients, families, co-workers and community agencies

  • Conduct psychosocial assessment of potential organ transplant living donors

  • Provide emotional support, financial referral, and concrete services to Organ Transplant patients and their families

  • Work as an integral part of the multidisciplinary Organ Transplant team in the inpatient and outpatient settings, pre- and post-transplant


Required qualifications for this position include:

  • Master's Degree in Social Work

  • Licensed Social Work Associate - Advanced (LSWAA) or Licensed Social Work Associate - Independent Clinical (LSWAIC)

  • Experience post Masters work in a medical setting

  • Experience working with issues of chronic illness and disability

Preferred qualifications for this position include:

  • Licensed Independent Clinical Social Worker (LICSW) or Licensed Advanced Social Worker (LASW) with the Washington State Department of Health

  • Substance abuse treatment knowledge and experience
