Swedish Medical Center is looking for a Social Worker (1.0 FTE, Days) to work in the Swedish Neuroscience Institute r at Swedish Cherry Hill Campus in Seattle, WA.

The Social Worker is responsible for providing medical related social services to patients and families. These services include psychosocial and discharge needs assessments, planning and intervention.This position would be a split between the Swedish MS Center (60%) and the ALS/ General Neurology Clinic (40%).

In this position you will:

Effectively

assess the needs of the client and the client’s family (when appropriate) and

arrange, coordinate, monitor, evaluate, and advocate for a package of multiple

services to meet the specific client’s complex needs.

Provide

assistance and advocacy to clinic patients in support of medical, financial

benefits and community based programs/services.

·

Collaborate

with providers and clinic staff on State, Federal and community systems,

applications, etc. being pursued by patients for their psychosocial needs.

·

Participate

in development and implementation of programs and activities designed to

educate the community and ensure availability of resources.

·

Execute

assigned administrative tasks relating to patient care or treatment plans.

·

Research,

develop and implement clinic, network, and/or community-wide programs; evaluate

programs and disseminate findings.

·

Maintain

knowledge of current trends and developments in the social work field.

·

Demonstrate

competency with all position related software programs.

Qualifications:

Required qualifications for this position include:

  • Master's Degree in Social Work

  • Licensed Social Work Associate - Advanced (LSWAA) or Licensed Social Work Associate - Independent Clinical (LSWAIC)

  • Experience post Masters work in a medical setting

Preferred qualifications for this position include:

  • Licensed Independent Clinical Social Worker (LICSW) or Licensed Advanced Social Worker (LASW) with the Washington State Department of Health

  • Experience working with issues of chronic illness anddisability
