Swedish Health Services Supervisor MSW Case Management - Care Coordination in Edmonds, Washington

Description:

Swedish Medical Center is looking for a Supervisor (MSW) (1.0 FTE, Days) to work in Case Management at Swedish Edmonds in Edmonds, WA.

The Supervisor position reports to the Manager, Case Management and is responsible for the direct supervision of assigned Social Workers. Supervision includes the daily coordination and management of all assigned Social Work staff to ensure excellence in customer service. The Supervisor develops and implements an operational plan that includes staffing resources, policy and procedures, orientation and education, and quality improvement activities under the direction of the manager. The position assures timely provision of coordinated patient care that includes all social work functions of care coordination, discharge planning, and psycho-social assessment and interventions through application of social work standards of care. The Supervisor maintains expertise in social service processes and ensures that services are aligned with the vision and goals of the department.

In this position you will:

  • Collaboration: Works collaboratively with patients nurses, social workers, physicians, other practitioners, caregivers and community resources and agencies. The case manager and healthcare team are jointly accountable for measurable outcomes that are cost effective and reflect patient preferences and values.

  • Communication: Communicates timely, relevant and accurate information to all parties involved with a patient’s care.

  • Facilitation: Facilitates the progression of care by advancing the care plan to achieve desired outcomes.

  • Coordination: Integrates the work of the healthcare team by coordinating resources and services necessary to accomplish agreed-upon goals.

  • Advocacy: Advocates on behalf of patients and caregivers for service access or creation, and for the protection of the patient’s health, safety and rights.

Qualifications:

Required qualifications for this position include:

  • Master's Degree in Social Work

  • 3 years recent experience dealing with diverse patient populations including various age groups with physical or developmental disabilities, emotional, behavioral, and mental disorders, and substance abuse, or complex care needs in acute inpatient care areas

  • Social Work experience

  • Current Washington State Licensed Independent Clinical Social Worker (LICSW) or Licensed Advanced Social Worker (LASW) with the Washington State Department of Health under Chapter 18.225 RCW required

  • Minimum 1 year in a licensed status

Preferred qualification for this position include:

  • Supervisor or lead experience
