Swedish Health Services Patient Care Navigator BSW in Seattle, Washington
Swedish Medical Center is looking for a Patient Care Navigator BSW (0.5 FTE, Days) to work in Family Medicine Clinic at Swedish Ballard in Seattle, WA.
Serves as the family medicine clinic's link to support services for patients, physicians, and staff alike and includes clinical knowledge relevant to housing, transportation, public benefits, medical legal advocacy, and other related resources. Supports patients, physicians, and staff by identifying and connecting patients with these resources within both the community and Swedish health system alike.
In this position you will:
Acts as liaison between the patient and the various clinic areas, hospitals, and ancillary services to coordinate activities involved for patient care.
Contacts and assists patients who are “at risk” for barriers to care, in coordination with and as directed by case manager and/or care team.
Coordinates multidisciplinary planning conferences, develops concise patient summaries for use by the care team, and documents recommendations made utilizing standardized care protocols.
Promotes and protects general safety for patients/families/friends and co-workers through adherence to correct procedures and timely communication of messages.
Communicate with health care team members regarding the status and/or completion of care coordination activities.
Explains topics of insurance coverage, treatment costs, and financial assistance opportunities to patients and/or family members during catastrophic or challenging life events. Such situations involve but are not limited to death, amputation and disability.
Required qualifications for this position include:
Bachelor's Degree in Social Work
Experience within a Health Related field
2 years in a patient care setting involving interaction with a variety of medical services, contact with patients and families, and interaction with physicians and providers.
Providing a high level of customer service including handling confidential patient health information (PHI) with discretion
CCM certification is a plus.
