Swedish Medical Center Pharmacist - Anticoagulation Therapy Services in Seattle, Washington


Swedish Medical Center is looking for a Pharmacist (0.9 FTE, Days) to work in the Anticoagulation Therapy Services at Swedish First Hill located in Seattle, WA.

We are seeking a detail focused, caring, and thoughtful individual in Pharmacy care, promoting the goals and upholding the ideals of Swedish Medical Center in providing excellent care and experience for our diverse patient population. This position is a member of a multi-disciplinary team and is responsible for assuring safe, efficient, cost effective drug therapy for all patients.

In this position you will:

  • Provideanticoagulation medication management services and clinical druginformation to meet the needs of the customers, including physicians, nurses,patients and other healthcare professionals

  • Participatein the training, orientation, and education of staff

  • Completeassigned projects and collaborate on initiatives intended to improve thequality of our anticoagulation services

  • Completeother tasks as assigned


Required qualifications for this position include:

  • Graduateof an accredited school of pharmacy

  • Currentlicense to practice as a registered pharmacist in the State of Washington

  • Abilityto accurately check the compounding of IV admixtures, HA solutions andother parenteral drugs, manufactured items, unit-dose medication andoutpatient prescriptions

Preferred qualifications for this position include:

  • Experiencein providing outpatient anticoagulation management services

  • Experiencein monitoring of service-related quality metrics

  • ASHP-accreditedPGY-1 residency or equivalent experience

  • BoardCertification in an applicable specialty
