Swedish Health Services Pharmacist - First Hill Cancer Pharmacy in Seattle, Washington

Where you work matters! Swedish Medical Center is looking for an inpatient pharmacist (1.0 FTE, variable shifts) with focus in oncology to work at Swedish First Hill inpatient pharmacy, Swedish Cancer Institute pharmacy located in Seattle, WA.

We are seeking a detail, focused, caring, and thoughtful individual in pharmacy care. Promoting the goals and upholding the ideals of Swedish Medical Center in providing excellent care an experience for our diverse patient population. This position is a member of multi-disciplinary team and is responsible for assuring safe, efficient, cost-effective therapy for all patients.

Essential Functions:

  • Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability and makes recommendations for therapy changes to the providers and facility staff as appropriate

  • Evaluate patient electronic medical records to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including over the counter and home remedies)

  • Collaborates with the healthcare team to provide clinical and technical support as needed

  • Documents all communication with physicians, nurses, other members of the healthcare team and patients and their family members in the electronic medical record

  • Provide pharmaceutical care that includes cost conscious therapeutic drug monitoring, pharmacokinetic evaluations, and documentation of patient medical records

  • Respond to pharmaceutical inquiries from patients, medical personnel, and pharmacy staff

  • Oversee activities and assign projects to resident pharmacist, interns, and other pharmacy personnel

  • Maintain responsibility for handling incoming provider calls and collaborating to resolve issues

  • Perform other tasks as assigned

Qualifications:

Required qualifications for this position include:

  • Graduate of an accredited school of pharmacy

  • Current license to practice as registered pharmacist in the State of Washington

  • Ability to accurately to process and check the compounding of IV admixtures, TPN, other parental drugs, chemotherapy, and other hazardous drugs

  • Ability to follow collaborative drug therapy agreement (CDTA) and pharmacy guidelines and protocols for appropriate monitoring and dosing of antimicrobial agents, anticoagulation, Glycemic control, and all relevant pharmacy service

Preferred qualifications for this position include:

  • Experience in hospital inpatient pharmacy with a focus in adult Oncology patient populations

  • ASHP-accredited PGY-1 residency or equivalent experience with focus in oncology and/or Oncology Board Certification
