Swedish Medical Center Pharmacist - Inpatient in Seattle, Washington
Swedish Medical Center is looking for anInpatient Pharmacist (0.875 FTE, Night shift) to work atSwedish First Hill in Seattle, WA.
We are seeking a detail focused, caring, and thoughtful
individual in Pharmacy care, promoting the goals and upholding the ideals of
Swedish Medical Center in providing excellent care and experience for our
diverse patient population. This position is a member of a
multi-disciplinary team and is responsible for assuring safe, efficient, cost
effective drug therapy for all patients.
Schedule: Night shifts / 7 on 7 off
In this position you will:
Evaluates physician medicationorders for appropriateness of drug, dosage, potential interactions, routeof administration and stability and makes recommendations for therapychanges to the providers and facility staff as appropriate.
Maintains patient database toinclude appropriate current and past medical history, appropriate clinicaland laboratory data, and concurrent medication (including over-the-counterand home remedies).
Collaborates with thehealthcare team to provide clinical and technical support as needed.
Documents all communicationwith physicians, nurses, other members of the healthcare team and patientsand their family members in the electronic medical record.
Provide pharmaceutical carethat includes cost conscious therapeutic drug monitoring, pharmacokineticevaluations, and documentation of patient medical records.
Respond to pharmaceuticalinquiries from patients, medical personnel, and pharmacy staff.
Oversee activities and assignprojects to resident pharmacist, interns and other pharmacypersonnel.
Responsible for, handlingincoming physician calls, logging data into the computer, and DrugUtilization Review.

Required qualifications for this position include:
Graduate of an accreditedschool of pharmacy
Current license to practice asregistered pharmacist in the State of Washington
Ability to accurately check thecompounding of IV admixtures, TPN and other parental drugs andchemotherapy drugs
Ability to follow collaborativepractice agreement (CPA) and pharmacy guidelines and protocols forappropriate monitoring and dosing of antimicrobial agents,anticoagulation, Glycemic control, and other services
Preferred qualifications for this position include:
- Experience in hospitalinpatient pharmacy
