Swedish Medical Center is lookingfor an Inpatient P harmacist (1.0FTE,variable shifts) with focus in oncology to work at Swedish First Hill inpatientpharmacy, Swedish Cancer Institute pharmacy located in Seattle, WA and rotationin Swedish Cancer Institute clinic pharmacy in Edmonds, WA.
We are seeking a detail, focused, caring, and thoughtful
individual in pharmacy care. Promoting the goals and upholding the ideals of
Swedish Medical Center in providing excellent care an experience for our
diverse patient population. This position is a member of multi-disciplinary
team and is responsible for assuring safe, efficient, cost effective therapy
for all patients.
Schedule: Days w/rotating evenings and weekends.
In this position you will:
Evaluates physicianmedication orders for appropriateness of drug, dosage, potentialinteractions, route of administration and stability and makesrecommendations for therapy changes to the providers and facility staff asappropriate
Evaluate patientelectronic medical records to include appropriate current and past medicalhistory, appropriate clinical and laboratory data, and concurrentmedication (including over-the-counter and home remedies)
Collaborates with thehealthcare team to provide clinical and technical support as needed
Documents allcommunication with physicians, nurses, other members of the healthcareteam and patients and their family members in the electronic medical record
Provide pharmaceutical carethat includes cost conscious therapeutic drug monitoring, pharmacokineticevaluations, and documentation of patient medical records
Respond to pharmaceuticalinquiries from patients, medical personnel, and pharmacy staff
Oversee activities andassign projects to resident pharmacist, interns and other pharmacypersonnel
Maintain responsibilityfor handling incoming provider calls and collaborating to resolve issues
Perform other tasks asassigned
To perform this jobsuccessfully, an individual must be able to perform each essential dutysatisfactorily. The requirements listed above are representative of theknowledge, skill, and/or ability required
Required qualifications for this position include:
Graduate of an accredited school of pharmacy
Current license to practice as a registered pharmacist in the State of Washington
Ability to accurately check the compounding of IV admixtures, HA solutions and other parenteral drugs, manufactured items, unit-dose medication and outpatient prescriptions
Ability to follow collaborativedrug therapy agreement (CDTA) and pharmacy guidelines and protocols forappropriate monitoring and dosing of antimicrobial agents, anticoagulation,Glycemic control, and all relevant pharmacy service
Preferred qualifications for this position include:
Experience in hospital inpatient pharmacy with a focus inadult Oncology patient populations
ASHP-accredited PGY-1 residency or equivalent experiencewith focus in oncology
Board certification in an applicable specialty
