Swedish Medical Center is looking for a Pharmacist (1.0 FTE, Variable shifts) to work at Swedish First Hill located in Seattle, WA.

We are seeking a detail focused, caring, and thoughtful individual in Pharmacy care, promoting the goals and upholding the ideals of Swedish Medical Center in providing excellent care and experience for our diverse patient population. This position is a member of a multi-disciplinary team and is responsible for assuring safe, efficient, cost effective drug therapy for all patients.

In this position you will:

  • Evaluateprovider medication orders for appropriateness of drug, dosage, potentialinteractions, route of administration and stability, and makerecommendations for therapy changes as appropriate

  • Evaluatepatient electronic medical records to include appropriate current and pastmedical history, appropriate clinical and laboratory data, and concurrentmedications (including over-the-counter and home therapies)

  • Collaboratewith the healthcare team to provide clinical and technical support asneeded

  • Documentrelevant communication with physicians, nurses, other members of thehealthcare team, patients and their family members in the electronicmedical record

  • Providecost-effective, evidence-based pharmaceutical care that includestherapeutic drug monitoring, pharmacokinetic evaluation, drug utilizationreview, use of collaborative drug therapy agreements and drug therapyguidelines

  • Respondto pharmaceutical inquiries from medical personnel, and pharmacy staff

  • Overseeactivities and assign projects to residents, interns, and other pharmacypersonnel

  • Maintainresponsibility for handling incoming provider calls and collaborating toresolve issues

  • Performother tasks as assigned

Qualifications:

Required qualifications for this position include:

  • Graduateof an accredited school of pharmacy

  • Currentlicense to practice as registered pharmacist in the State of Washington

  • Abilityto accurately check the compounding of IV admixtures, TPN, and otherparental drugs

Preferred qualifications for this position include:

  • Experiencein hospital inpatient pharmacy with a focus on interdisciplinary care

  • ASHP-accreditedPGY-1 residency or equivalent experience

  • Abilityto follow a collaborative drug therapy agreement (CDTA) and pharmacy drugtherapy guidelines for all relevant pharmacy services including but notlimited to anticoagulants (parenteral and enteral), antimicrobials, parenteralnutrition, and antiepileptics

  • Boardcertification in an applicable specialty
