Swedish Medical Center is looking for a Pharmacy Technician (1.0 FTE, Days) at Swedish First Hill in Seattle, WA.
Swedish Medical Center has an outstanding opportunity for a
Pharmacy Technician with a focus on accurate patient medication histories to
join our team! The Medication History Pharmacy Technician will assist the
pharmacist by performing the following medication history activities, with the
goal of compiling an accurate and complete medication list prior to hospital
Collectionand clarification of the home medication list through patient or caregiverinterview, information gathered from electronic sources, communitypharmacies and primary care provider.
Verificationof allergy information.
Entryof the home medication list into the electronic health record (EHR), aswell as updating the patient’s preferred community pharmacy information inthe EHR.
Documentationin the EHR and communication of relevant information associated with homemedications in the EHR and/or verbally to the pharmacist.
Communicatingconcisely and effectively with patients, families, pharmacists, and otherhealth care providers related to patient home medication histories.
The Medication History Pharmacy Technician will also be
cross-trained in other Inpatient Pharmacy areas to support coverage as needed,
however the primary work location will be within the Emergency Department.
Required qualifications for this position include:
HSDiploma or equivalent
Completionof an approved pharmacy technician training program
Currentlicense to practice as a pharmacy technician in the State of Washington
Excellentcomputer, customer service, and written/verbal communication skills.
Accuratework performance with an attention to detail.
Proactivelysets priorities and solves problems.
Preferred qualifications for this position include:
Twoyears’ experience as a Certified Pharmacy Technician in a hospital orcommunity pharmacy setting.
Broad pharmacy experiencein different settings (e.g. community, hospital, home health, etc.)
Prior experience withobtaining medication histories and/or medication reconciliation
Working knowledge ofcommon outpatient medications, doses and forms
