Swedish Health Services Referral and Scheduling Coordinator - Diabetes Education Center in Seattle, Washington
Swedish Medical Center is looking for a Referral and Scheduling Coordinator (1.0 FTE, Day Shift) to work in Diabetes Education Center at Swedish First Hill in Seattle, WA.
This position works under the general direction of a physician or their designee, utilizing communication and management skills to coordinate seamless care delivery for patients in a surgical or non-surgical setting. Will act as a patient advocate to assure the patient's access to health care during their entry into and through the services of the clinic.
In this position you will:
Acts as a contact for clinic providers, patient and family, referring physicians, outside agencies, outside staff and general public by providing information regarding the clinic’s services.
Acts as a liaison between the patient and the various clinic areas and ancillary services to coordinate activities involved in pre-operative planning and preparation or planned hospitalization.
Coordinates pre-admission tests, pre-surgery appointments/tests, and procedures as delegated by the provider or nurse.
Ensures a complete medical record is present at the time of patient visit, admission, and/or procedure.
Facilitates the flow of documents between patient, providers and payers.
Required qualifications for this position include:
High School Diploma or GED
One (1) year of work experience in a tertiary patient care setting involving interaction with a variety of medical services, contact with patients and families, and interaction with physicians
Preferred qualifications for this position include:
Bachelor’s degree in Health Related field
N.A.C. (Certified Nursing Assistant) certification
