Swedish Health Services RN Case Manager - Collaborative Health Action Team in Seattle, Washington


Swedish Medical Group is looking for a Case Manager RN (1.0 FTE, DAY SHIFT) to work in Collaborative Health Action Team at Swedish Medical Group in Seattle, WA.

The RN Case Manager evaluates and manages the changing health and service needs of the patient. The Case Manager RN is responsible for providing optimum individual care for the patient/family unit, using the nursing process and interdisciplinary team members. Collaborates with the patient's Primary Care Physician and other health team members to create and implement a written care plan within the time frame identified in documentation standards.

In this position you will:

  • Develops the initial individualized care plan based on diagnoses and clinical assessment within the time frame identified in guidelines, in collaboration with the Primary Care Provider (PCP). Collaborates with PCP regarding individual case issues and changes in plan of care.

  • Establishes expected outcomes that are measurable and achievable and provides follow up to client, PCP, and health care team.

  • Evaluates patient’s response to care and initiates independent and/or collaborative changes when appropriate. Documents changes in condition and interventions.

  • Completes all documents necessary to reflect case management activities provided to each client. Keeps registry up-to-date and relevant.

  • Engages in encouraging and effect communication with patients, families, physicians, and coworkers. Acts as a patient advocate and uses resources effectively.


Required qualifications for this position include:

  • Current Washington State RN license

  • Basic Life Support (BLS) for Health Care Providers required at time of hire from American Heart Association (AHA) or Military Training Network (MTN)

Preferred qualification for this position include:

  • Bachelor's Degree in Nursing (BSN)

  • Three (3) years current experience in a health care environment with home health case management experience
