Swedish Health Services RN Navigator Stroke Program in Seattle, Washington
Swedish Medical Center is looking for a RN Navigator Stroke Program (1.0 FTE, Days) in Neuroscience Stroke Clinic at Swedish Cherry Hill in Seattle, WA.
Relocation and a 10K sign on bonus is availabe for this position.
5k at hire and 5k after one year
The RN Stroke Navigator will contact the patient before hospital discharge, and then follow the patient after discharge providing coordination between the patient, PCP, neurologist, and other specialists. This position will perform in-hospital and early post-discharge screenings for medication compliance, healthy life style, depression, medical complications, and caregiver burden while being an advocate and anticipating their needs.
In this position you will:
Provides education and care coordination for stroke patient populations with an emphasis on just-prior-to-discharge and post-discharge care.
Supports appropriate therapy based care models for defined patient populations with direction from program physicians
Coordinate, manage, and obtain necessary consultations and assist in admission and discharge plans of patients in the Swedish Health System
Participate in stroke patient hospital rounds when appropriate and work with the provider teams to ensure appropriate discharge education and coordination with inpatient services.
Teaches, supervises and counsels the patient and caregivers regarding nursing care and other related problems of the patient in the home. Provides education regarding medication, symptom management, health teaching, and anticipatory guidance to patients and their families
Required qualifications for this position include:
Bachelor's dergree in Nursing
Current Washington RN license required.
5 years related neurological clinical experience.
NIHSS certification required within 30 days of hire
Modified Rankin certification required within 30 days of hire.
Eight hours of stroke education required annually.
Preferred qualifications for this position include:
Master's Degree in Nursing
1 year experience in an Ambulatory setting.
Experience working with patients who have neurological conditions, and their families.
Experience working in a certified Comprehensive Stroke Center.
Experience leading formal educational forums/in-services.
Stroke Certified Registered Nurse
American Association of Neuroscience Nursing membership and certification
