Swedish Medical Center Senior Patient Care Coordinator - Heart And Vascular Clinic in Seattle, Washington

Description:

Swedish Medical Group is looking for a Senior Patient Care Coordinator (1.0 FTE, Day Shift) to work in Heart and Vascular Clinic at Swedish Cherry Hill in Seattle, WA.

The Senior Patient Care Coordinator works under the general direction of a physician or his/her designee, utilizing communication and management skills to coordinate seamless care delivery for patients in a surgical or non-surgical setting, and provides basic patient care to assigned patients and act as a patient advocate to assure the patient’s access to care during their entry into and through the services of the clinic.

In this position you will:

  • Responsible for the daily operations and planning of workflow for the Patient Care Coordinators (PCC) to ensure that all team members know what their priorities are, daily

  • Assist with scheduling surgeries and/ or procedures or services needed in the ambulatory management of the patient's care.

  • Ensure a complete medical record is present at the time of patient visit, admission, and/ or procedure

  • Responsible for evaluating the efficiencies of the team as well as identifying barriers and move to eliminate them where possible

  • Acts as the liaison between the patient and the various clinic areas and ancillary services to coordinate activities involved in pre-operative planning and preparation or planned hospitalization

  • Perform other duties as assigned

Qualifications:

Required qualifications for this position include:

  • High School diploma or GED

  • Minimum of 2 years' experience

  • Demonstrated PC skills, i.e. proficient with MS Office products including Word, Excel, and Outlook

Preferred qualifications for this position include:

  • Electronic Medical Records (EMR) experience

  • Medical Assistant Certification

  • Previous experience in a healthcare setting

  • Prior experience providing a high level of customer service in a fast paced environment, including handling confidential data with discretion

  • Solid understanding of medical terminology
